1Employees, spouses, and adult dependents must verify their eligibility using the employee's ID and their own first name, last name, and date of birth.
2After verification, employees, spouses, and adult dependents must create individual accounts using their own email address.
3Benefits for minor dependents will already be associated with the employee’s account after verification/account creation.
